At the moment, the solution appears to be that you should replace the video card via warranty if it fails within warranty period, and eventually retire the video card for a newer design (which hopefully better handles/avoids the problem). Each time one of those spikes occurs, there is a chance that the VRM will fail. The problem is that the NVIDIA power monitoring design is not designed to prevent momentary power spikes which exceed the VRM's capability. Blame whatever you want, but if the design issue didn't exist, the card wouldn't fail. These cards have an underlying design issue.
